Well, tolerances that tight for clearance is poor design to me!

Sounds suspicious to me that different truck frames or wheel wheels sets 
came from different die cavities. Would me interesting to measure a 
selection of wheel sets over the flanges to see the variaton.

But these things happen to the best of companies (speaking from experience 
again!)
