Regarding the question about what size was used in homes, I believe the
answer is: it depends. I know my Dad always ordered Chestnut coal for our
furnace, which was a manually fed, manually operated beast built around
1900. 

However, some of those in "newer" homes or those who were better off (like
my friends in the Green Ridge section of Scranton) had furnaces with
automatic feeds and dampers. If memory serves me correctly, their furnaces
used rice or pea coal.
