Mike raises some interesting points, but we should also keep in mind photo manipulations were possible long before digital imaging. Just look at the recent revelations about the Loch Ness Monster where the photos, from the 1930's, I believe, were faked. The difference today is that you do not have to be a darkroom wizard such as Ansel Adams to manipulate the image. Anybody with a computer and some cheap photo editing software can do it.
