Dover Harbor is an exception:  it has been heavily rebuilt to be both comfortable and Amtrak compatible as a charter car.  The owners (Washington, DC, chapter, NRHS) have been very careful to retain the 1930s flavor and floor plan while upgrading electricity, plumbing and heat for modern use.  I'm sure that the chapter added the shower during renovation.

Showers were quite rare in regular train service, because they were wasteful in a time when each car carried its own water.
