The same thing happened at EL Marion interchange with the PC (old PRR) in Jersey City by West End tower in New Jersey.  
 
Cadillac stopped making convertables in 75 or 76 and PC EL crews placed tri levels on one of the tracks that wasn't lowered to accomodate tri levels.  The EL boys just started pulling and  made 30 brand new convertables when two carloads had the upper levels roofs sheared off.
 
And of course no one could talk to each other because the radio frequencies weren't the same and in those days the radios didn't have other channels to turn too.

train with autoracks detouring on the B&O in Ohio due to a derailment on PC.
Evidently someone didn't check the clearance diagrams carefully enough, as
the racks hit a low underpass, trashing about a million bucks worth of
cadillacs.
